Referrals will be accepted from any patient or service user, relative or health professional, but can only be made with the person’s consent.
If you are a person requiring care or support, you can either speak to your nurse, doctor or relevant health care professional about making a referral, or alternatively you can make a referral yourself by downloading the form and emailing it to: pendlesidehosp.referrals@nhs.net

When referrals are made directly by patients or their relatives/carers, we will always, with consent, contact the patient’s General Practitioner or other relevant healthcare professionals to gather the clinical information needed to deliver appropriate care and support. All our services are available to residents of Burnley and Pendle. Rossendale residents can also access our inpatient unit service.
