Make a Referral


Referrals are accepted from patients, relatives, carers and healthcare professionals.  

You may complete the referral form and email the completed form to: pendlesidehosp.referrals@nhs.net or contact the Hospice on 01282 440100 if the referral is urgent or you are unable to complete and email the form. 

It is important that the patient consents to the referral before it is completed. We may need to contact relevant health or social care professionals to obtain more clinical information. 

What happens next? 

When a referral has been received, it will be discussed by the Hospice multi-disciplinary team and prioritised according to each individual need. A member of the relevant clinical team will then make contact with the referrer and patient to discuss next steps, including seeking further information where required.  

Geographical area 

All our services are available to residents of Burnley and Pendle; Rossendale residents can also access our inpatient service.
