The multidisciplinary team of Healthcare professionals and volunteers, deliver group sessions for palliative care, neurological & respiratory conditions, dementia, and frailty, alongside social and therapeutic activities such as art, music, and upcycling in the Shabby Shed. We also provide psychological support after diagnosis, rehabilitation (physiotherapy and occupational therapy) and our Complementary Therapists support the wellbeing of patients and carers.
Self-referrals are welcome, as well as those from relatives, carers, and healthcare professionals. Once a referral has been received, a member of the team will contact the patient and/or carer to discuss and arrange the next steps.
